The U.S. Coast Guard has released revised versions of the forms CG-719 K (Merchant Mariner Credential Medical Evaluation Report) and CG-719 K/E (Merchant Mariner Evaluation of Fitness for Entry Level Ratings). The new forms are considerably longer and more detailed than the previous forms.
Mariners may use the previous four-page medical form until Dec. 31, 2009,after which the new forms will be mandatory. Mariners may also begin voluntarily using the new forms now. All physical exams completed on or after Jan. 1, 2010, must be documented on the new form.
