The AMO Medical Plan will no longer require an official registrar’s receipt or official school transcript when completing the Affidavit of Support for Dependent Student form.
A letter explaining this change has been mailed to all AMO Medical Plan participants with eligible dependent students between the ages of 19 and 23. The complete letter is available in the “AMO Plans Bulletins” section of the AMO Plans Web site.
If you have an eligible dependent student, please complete the revised form, have it notarized and return it no later than September 1, 2009. Please read the document carefully before signing. The form, “2009 AMO Medical Plan COB & Affidavit of Support for Dependent Student,” is available in the “Medical Plan Forms” section of the AMO Plans Web site.
