Beginning January 1, 2015, American Maritime Officers members and applicants will be using a new form to apply for benefits from the AMO Vacation Plan.
When signed and submitted, the new application authorizes the AMO Vacation Plan to deduct from the vacation claim any amount owed by the member or applicant who submitted it to become current in AMO union dues and/or initiation payments. Any dues or initiation payments deducted from a claim will be transferred to the union so the member or applicant will become current at that time.
The new application for vacation benefits eliminates the requirement for AMO members and applicants to complete a separate authorization form to have outstanding dues and initiation payments deducted from a vacation claim.
AMO members and applicants will still need to complete a separate authorization form to have a contribution to the AMO Voluntary Political Action Fund deducted from a vacation claim.
The new application form will be available on the AMO Plans website in January 2015 and will be distributed to AMO members and applicants requesting an application for vacation benefits at that time.
Above the signature line, the new application form states: "I hereby authorize and direct the American Maritime Officers Vacation Plan to deduct from vacation benefits due me all monies due and payable by me as initiation fees and dues to American Maritime Officers."
The following disclaimer is required to be printed on each form: "While dues/initiation fees to American Maritime Officers are not taxable for Federal Income Tax purposes; they may be Tax Deductible under other provisions of the Internal Revenue Code."
