Defined Contribution Plan 2011 percentage determination statements, additional plan information mailed to AMO members


AMO members will be receiving their AMO Defined Contribution Plan 2011 Percentage Determination Statement this week along with additional Plan information, including the investment options in which Plan participants may invest their accounts.

You are asked to review the statement and if you agree with the information provided, sign and return the statement to the AMO Plans Benefit Services Department no later than June 30, 2011. If you do not agree with the information provided, indicate your changes on the statement and return it to the Benefit Services Department, along with supporting documentation, no later than June 30, 2011.

If you do not return the statement by June 30, 2011, the percentage determination on the statement will be deemed as approved by you and contributions to your account will be made accordingly.

In addition to the mailed package, you may also view your AMO Defined Contribution Plan 2011 Percentage Determination Statement securely online by logging on to your Newport Group account. Once you have logged in:
  1. Select your Defined Contribution Plan account.
  2. Select the "My Account" menu.
  3. Under the "My Account" sub-menu select "Statements"
  4. Under the "Current Statement" section select the link titled 01/01/2011 - 12/31/2011. A new window will appear with your Defined Contribution Plan statement in Adobe PDF format.
If you have not yet logged on to your Newport Group account you will need to establish a login ID and a personal identification number (PIN).
  • The default login ID is your social security number with no dashes, for example, 123456789.
  • The default PIN is the month and last two-digit year of your birth date. For example, if your actual birth date is March 6, 1962, your pin would be 0362.
